Monograph of Rauwolfia serpentina
3.1 History
The plant Rauwolfia is named after the German doctor and traveller Leonhard Rauwolf, who in 1582 published an account of his many travels. Roots of the plant Rauwolfia serpentina (Benth) have been recognized in India and the Malay Peninsula from ancient times as antidotes to the stings and bites of insects and poisonous reptiles. Mention of the plant is found in an old Hindu manuscript (1000 B.C.) as well as in the monumental works of Charaka (second century A.D.), under the Sanskrit name of “sarpagandha.” It has also been used as a febrifuge, as a stimulant to uterine contractions, for diarrhoea, dysentery, insomnia, and insanity. For just over two decades, its clinical application has been extended with success to the treatment of high blood pressure.
The Sanskrit word ‘sarpagandha’ literally means one that smells like a serpent. It is mentioned in all ancient Ayurvedic scriptures for its special property, as a sedative. It is also cited to have a special potency as an anti-epileptic and as a remedy for hysteria. Maharishi Charka has categorized it as svapna janana - sedative Sarpagandha. During the last few decades, it has attracted the attention of scientists for its anti-hypertensive property. Hindus used this plant for centuries as a febrifuge and as an antidote to the bites of poisonous reptiles like snakes. It was also used to treat dysentery and other painful affections of the intestinal canal. Some believed it caused uterine contraction and promoted the expulsion of the fetus. It was also mentioned as a stomachic that cures fever. In the western parts of India, the root was used as a remedy for painful affections of the bowels. With Aristolochia, it was given to cure cholera; with Holorrhena antidysenterica and Jatropha curcus given in colic; with Andrographis paniculata, it was given for fever. In the last decade, its medicinal value has been accepted by the allopathic system.
The Hahnemannian pathogenesis was done by Otto Leeser and R. Schrenk in 1954 in the Robert Bosch Hospital of Stuttgart, Germany. The experiment was done on 24 doctors with the dynamisations 1DH, 3DH for a duration of 14 to 28 days. W. L. Templeton has also done a proving in the homoeopathic faculty in London in 1954-55, with 6 provers and with the dynamisations 3DH, 12CH, 30CH according to the provers, for 6 months. Beja d’ Orleans (who seems not to know the proving of O. Leeser) has done an auto experiment in July-August, 1956 with the dynamisation 3 DH, 10 drops a day for one month. Finally, Julian has done minute clinical studies in order to fix the field of action of Rauwolfia in homoeopathic dilutions.
Contraindications
- Radix Rauwolfiae products are contraindicated in patients who have previously demonstrated hypersensitivity to the plant or its alkaloids.
- They are also contraindicated in patients with a history of mental depression (especially those with suicidal tendencies) during or shortly after therapy with monoamine oxidase inhibitors; active peptic ulcer, sinus node disorders, ulcerative colitis; epilepsy; or decreased renal function; and in patients receiving electroconvulsive therapy.
5. Warnings
- Radix Rauwolfiae products may cause mental depression.
- Recognition of depression may be difficult because this condition may often be disguised by somatic complaints (masked depression). The products should be discontinued at first signs of depression such as despondency, early morning insomnia, loss of appetite, impotence, or self-deprecation.
- Drug-induced depression may persist for several months after drug withdrawal and may be severe enough to result in suicide.
- Sensitivity reactions may occur in patients with or without a history of allergy or bronchial asthma.
- The use of Radix Rauwolfiae products may impair alertness and make it inadvisable to drive or operate heavy machinery.
6. Precautions
6.1 General
- Because Radix Rauwolfiae preparations increase gastrointestinal motility and secretion, they should be used cautiously in persons with a history of peptic ulcer, ulcerative colitis, or gallstones where biliary colic may be precipitated.
- Persons on high doses should be observed carefully at regular intervals to detect possible reactivation of peptic ulcer.
- Caution should be exercised when treating hypertensive patients with renal insufficiency since they adjust poorly to lowered blood-pressure levels.
6.2 Drug interactions
- When administered concurrently, the following drugs may interact with or potentiate Radix Rauwolfiae and its alkaloids: alcohol or other central nervous system depressants, other antihypertensives or diuretics, digitalis glycosides or quinidine, levodopa, levomepromazine, monoamine oxidase inhibitors, sympathomimetics (direct-acting), and tricyclic antidepressants.
- Concomitant use of Radix Rauwolfiae products and anaesthetics may provoke a fall in blood pressure and add to the -adrenoceptor-blocking activity of propranolol.
6.3 Drug and laboratory test interactions
- Chronic administration of Radix Rauwolfiae preparations may increase serum prolactin levels and decrease excretion of urinary catecholamines and vanilmandelic acid. Therefore, any diagnostic tests performed for these determinations should be interpreted with caution.
- Radix Rauwolfiae preparations slightly decrease absorbance readings obtained on urinary steroid colorimetric determinations (e.g., modified Glenn-Nelson technique or Holtorff Koch modification of Zimmermann reaction), and thus false low results may be reported.
- Preoperative withdrawal of Radix Rauwolfiae products does not necessarily ensure circulatory stability during the procedure, and the anaesthetist must be informed of the patient’s drug history.
- Caution is indicated in elderly patients and also in those suffering from coronary and cerebral arteriosclerosis. Administration of products including Radix Rauwolfiae preparations at doses that might precipitate a sharp decrease in blood pressure should be avoided.
6.4 Carcinogenesis, mutagenesis, impairment of fertility
- Animal carcinogenicity studies using reserpine at doses 50 times as high as the average human dose have been conducted with rats and mice.
- Carcinogenic effects associated with the administration of reserpine include an increased incidence of adrenal medullary phaeochromocytomas in male rats, unidentified carcinomas of the seminal vesicles in male mice, and mammary cancer in female mice; carcinogenic effects were not seen in female rats.
- Bacteriological studies to determine mutagenicity using reserpine showed negative results. The extent of risk to humans is uncertain.
6.5 Pregnancy: teratogenic effects
- Reserpine, the major active alkaloid in Radix Rauwolfiae, administered parenterally, is teratogenic in rats at doses up to 2 mg/kg and to have an embryocidal effect in guinea pigs at 0.5 mg daily.
- There are no adequate and well-controlled studies in pregnant women.
6.6 Pregnancy: non-teratogenic effects
- Increased respiratory secretions, nasal congestion, cyanosis, hypothermia, and anorexia have occurred in neonates of mothers treated with Radix Rauwolfiae.
- Therefore, the use of Radix Rauwolfiae is not recommended during pregnancy.
6.7 Nursing mothers
- Rauwolfia alkaloids are excreted in human milk.
- Because of the potential for serious adverse reactions in nursing infants, use of Radix Rauwolfiae during lactation is not recommended.
6.8 Paediatric use
Safety and effectiveness in children have not been established.
Adverse Reactions
The following adverse reactions have been observed, but there are insufficient data to support an estimate of their frequency. The reactions are usually reversible and disappear when the Radix Rauwolfiae preparations are discontinued.
- Cardiovascular system: bradycardia, arrhythmias, particularly when used concurrently with digitalis or quinidine, angina-like symptoms. Water retention with oedema in persons with hypertensive vascular disease may occur rarely, but the condition generally clears with cessation of therapy, or the administration of a diuretic agent. Vasodilatation produced by rauwolfia alkaloids may result in nasal congestion, flushing, a feeling of warmth, and conjunctival congestion.
- Central nervous system: sensitization of the central nervous system manifested by optic atrophy, glaucoma, uveitis, deafness, and dull sensorium. Other reactions include depression, paradoxical anxiety, nightmares, nervousness, headache, dizziness, drowsiness. Large doses have produced parkinsonian syndrome, other extrapyramidal reactions, and convulsions.
- Gastrointestinal system: hypersecretion and increased intestinal motility, diarrhoea, vomiting, nausea, anorexia, and dryness of mouth. Gastrointestinal bleeding has occurred in isolated cases.
- Respiratory system: dyspnoea, epistaxis, nasal congestion.
- Hypersensitivity: purpura, pruritus, rash. Other: dysuria, muscular aches, weight gain, breast engorgement, pseudolactation, impotence or decreased libido, gynaecomastia.

Homoeopathic Uses of Rauwolfia Serpentina
The hypnotic action of the drug appears to have been known to the poorer classes in Bihar, and the practice of putting children to sleep by this drug is stated to be still present in certain parts of that province. In Uttar Pradesh and Bihar, this drug is sold as “Pagal-ka-Dawa” (insanity specific), and its use is common amongst the practitioners of indigenous medicine. (Chopra, 1994) [5].
Sen and Bose (1931) [28] studied the pharmacological action on higher animals such as cats. They found that the watery extract of the whole drug, when injected intravenously in animals, produces no appreciable effect. The resins have also been separately tried but without effect on the system, except for slight stimulation on uterine musculature. The alkaloid isolated from them, however, showed definite results. The drug has been tried by Sen and Bose in cases of insanity with violent maniacal symptoms and in cases of high blood pressure. Doses of 20-30 g of the powdered root twice daily produce not only sedative effects but also a reduction of blood pressure. Within a week, the patient’s senses are said to be restored, though in certain cases, the period of treatment has to be prolonged. In high blood pressure without marked atheromatous changes in vessels, these authors find the drug very satisfactory.
Gupta and co-workers (1943) [11] treated fifteen patients suffering from various types of mental disorder with the standardized extract of R. serpentina Benth. Of these, five suffered from disorder of the affective reaction type, seven from schizophrenia, two from organic psychoses, and the remaining one from chronic epilepsy. All of these patients suffered from insomnia and showed considerable motor and mental agitation. The epileptic patient had less frequent major and minor fits. The extract was given daily at noon and at bedtime, usually in 30 minim doses. Sleep ensued 2-3 hours after the evening dose and lasted for more than 6 hours. The patients became quiet and behaved more normally, and those suffering from affective disorders showed considerable improvement. In the epileptic, the major fits were stopped, and the minor fits were controlled with great improvement. The alimentary functions of the patients were stimulated, and appetite improved.
Gupta, Kahali, and Dutta (1944) [12] found that a resinous, non-alkaloidal fraction from the Dehradun variant plant exerted the characteristic sedative action for which rauwolfia preparations are used clinically in India and suggested that for this purpose, the resin freed from alkaloids should be used.
Chakravarty and Chaudhury (1951) [4] treated 15 cases of essential hypertension with R. serpentina alkaloid. A definite fall of systolic pressure (15 mm or more) and diastolic pressure (10 mm or more) was seen on the same day after treatment in 80% and 100% respectively, and on the last day of treatment in 70% and 53.3% respectively. The maximum fall noticed was 53 mm systolic and 30 mm diastolic. Two patients showed transient untoward symptoms such as nausea, fainting, and dyspnoea during treatment.
- The mechanism of action of rauwolfia root differs from most other blood-pressure lowering agents, acting on the central nervous system. This may explain why it works when other medicines fail (Weiss, 1988; Shibuya and Sato, 1985) [37, 30].
- In doses higher than those used for hypertension, rauwolfia alkaloids cause a depletion of norepinephrine, resulting in a tranquilizing effect. Very high doses can cause a loss of coordination (reported in Huang, 1999) [16].
- Many patients who take medication to control hypertension still have problems with balance due to difficulties in circulatory regulation. Upon examination of blood-pressure lowering agents available up to 1980, researchers discovered that only rauwolfia alkaloids and clonidine do not have an undesirable influence on balance (Teichmann and Vogel, 1980).
- In a Chinese study on 200 patients with moderate hypertension, rauwolfia alkaloids lowered blood pressure by as much as 30-40% with minimal side effects (reported in Huang, 1999) [16].
- Rauwolfia root has proven highly effective (89%) in cases of chronic hives (reported in Huang, 1999) [16].
- The pharmacological effects of reserpine were formerly cause for concern that it might promote breast cancer. However, in epidemiological studies, rauwolfia alkaloids did not increase the risk of breast cancer (Shapiro et al., 1984; von Poser et al., 1990).
- Rauwolfia root has occasionally proven effective in cases of malnutrition that were unresponsive to high protein or high-energy diets (reported in Huang, 1999) [16].
Results and Final Discussions
